Important Instructions

1 Essential Instructions for Online Birth Registration

✓ On the website, completely fill out the prescribed form.
✓ Upload the mother's birth certificate (if available).
✓ Upload the mother's national identity card.
(If the mother is under 18 years of age, upload the "B Form".)
✓ After filling and submitting the online form, after three (3) days, come to the record room to obtain the extended birth certificate.

2 Instructions for Receiving Birth Certificate (Polyclinic)

✓ On the website, download the prescribed PDF form, print it (on both sides), and fill out the complete form.
✓ Also complete the "Statement of Constituency" (a stamp is required).
✓ Staple a copy of the mother's birth certificate (if available).
✓ Staple a copy of the mother's national identity card.
(If mother is under 18, staple the "B Form".)

Note: Bring the complete set of the above-mentioned documents with you and obtain the extended identity/personality from the record room. In case of incomplete documents or provision of incorrect information, there will be a delay in obtaining the birth certificate. The full responsibility for this will fall on the applicant.
